Initial Comments:
Based on the findings of an unannounced on-site follow-up survey completed on October 27, 2021, Westarm Homecare, was found to have corrected the deficiencies and to be in compliance with the requirements of 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of an onsite State Re-licensure and Medicare Re-certification survey completed on June 7, 2021.
